• Application Form

• Applicant’s Passport and a copy (Bio Page, used pages, arrival card for entry to Macau)

• Document justifying your extension of stay (relative’s documents, medical certificate, etc.)

• If a representative is applying on behalf of the applicant, there should be a certified authorization letter and a copy of valid ID

• This is free, without charge
