DFA Passport Guide: How to Change your Philippines Passport Maiden Name to your Married Name
Getting married to the love of your life is blissful and a fantastic feeling. We can’t wait for that honeymoon or spending the rest of our lives together. We can’t wait to change our surname after marriage. All our IDs need to correspond to our married name too.
For our Philippine passport, it is optional to change it if it is still valid. However, if we want everything to be the same, passport renewal is an easy process. If you are new, you can read this article.
Here are the requirements for passport renewal due to change of name due to marriage as per Department of Foreign Affairs:
Personal appearance
Confirmed appointment
Duly accomplished application form
Current Passport with photocopy of the bio-data page
Original PSA authenticated Marriage Contract
Valid IDs with one (1) photocopy – if possible with Married Name
Social Security System (SSS) / Government Service Insurance System (GSIS) Unified Multi-Purpose Identification (UMID) Card
Land Transportation Office (LTO) Driver’s License. Student Permit may be accepted if in card format.
Professional Regulatory Commission (PRC) ID
Overseas Workers Welfare Administration (OWWA) E-Card
Commission on Elections (COMELEC) Voter's ID or Voter's Certification from the Election Officer with Dry Seal
Philippine National Police (PNP) Firearms License
Senior Citizen ID
Airman License
Philippine Postal ID
School ID
Valid or Latest Passport (For Renewal of Passport)
Will my IDs with my maiden name be accepted?
As per Department of Foreign Affairs; Yes, you may still use your IDs with your maiden name in applying for a passport if you are newly married (within the last 6 months from date of appointment), but you need to supply an NBI clearance under your married name as a supporting document.

Before you start your renewal process, you should be able to access your e-mail as the form is submitted there. If you don’t have one or forgot your password, make a new one.
Here are the steps for your DFA’s Passport System:
Schedule an appointment
Go to passport.gov.ph.
Read all the reminders carefully before agreeing to the Terms and Conditions of the Online Appointment System.
Choose the nearest DFA Consular Office to your current address
Pick the date and time you are comfortable with to renew your passport. See your schedule.
Fill out all the information asked. Make sure the name you typed is your Married Name already, baka makalimutan mo.
Before clicking submit, review all the information you typed
Choose between expedited or regular then proceed to payment.
Open your e-mail to get your reference number.
Pay your processing fee at the Payment Center

For expedited processing, you will pay PHP 1,200.00 and for regular, PHP 950.00. A PHP 50.00 convenience fee is charged by authorized Payment Centers on top of the processing fees.
Give your reference number at your chosen payment center and pay the fee
One reference number = one transaction. Pay separately if you have multiple reference numbers.
Keep the issued receipt.
Before, the payment was after an appointment code has been sent to you. However there were issues about people reserving the slot and not paying the fee.Sayang naman ang slot if hindi ka pupunta, may taong need talaga ang appointment. Some fixers also reserve the slots and give it to those who need their services.

What to do on the day of your appointment
Double-check your documents if you have brought them all. Originals and Photocopies.
Be at the DFA office 30 minutes earlier than your appointment
Wear decent clothing when going to the office. Avoid slippers or shorts too. Sleeveless clothes, spaghetti, see-through, sando, tube, halters and the like are not allowed. Please remove jewelry (earrings, necklaces, and colored contact lenses) during picture taking.
Present your documents (if ever there are corrections, tell the processor about it)
After your Philippine Passport Renewal
Bring your passport receipt on the release of passport, in case of lost receipt, you have required an Affidavit of Loss
Check if all data is correct in the passport, bagong pangalan mo, please check
Affix signature on the third page of the passport booklet upon receipt of passport
If you can’t get your passport; an immediate family member can get it, or you need a Special Power of Attorney. Include a photocopy of your ID and the authorized person.
Take good care of your passport, don’t staple anything.

1. I made a mistake in filling out my application form. Will this affect my application? Will my appointment be canceled?
You can tell the processor during your appointment.
Mistakes may result to delay in your application. Any misrepresentation (pretending to be someone else or lying about circumstances) can be a reason for cancellation or refusal of appointment.
2. Can I reschedule my appointment? How do I reschedule my appointment?
Yes, you can reschedule your appointment; click the reschedule button. Please note that:
You can only reschedule weekday appointment to another weekday;
You can only reschedule once; and
Rescheduling is subject to the availability of appointment slots.
3. Can I cancel my appointment?
Yes, you may cancel your appointment here. Please be reminded that by canceling your appointment you forfeit your payment.
4. I won’t be going to my scheduled appointment, can I get a refund?
No, a refund can’t be processed.
5. I haven’t claimed my passport for months, is it still there?
It depends, if it hasn’t passed 180 days from the release then it is still at the DFA office you applied at. If 180 days have passed, your passport has been canceled.
6. My visa is in my old passport, but my new one has my married name, is it still valid?
Yes, please bring your old and new passport along with a Marriage Certificate.
